Primary Health Centre (PHC) Operational Capability Report 2026

Primary healthcare is the first point of contact for millions of Nigerians, yet many primary healthcare centres lack the basic conditions needed to provide safe and reliable care. The findings from the assessment show serious gaps in infrastructure, power, water, staffing, equipment, and accessibility.

Orodata conducted the assessment between October 2023 and June 2025 using the CheckMyPHC Digital Scorecard. Trained data collectors and journalists physically visited 1,480 PHCs across 16 states and 277 LGAs, with representation from all six geopolitical zones. Rural facilities made up about 75% of the assessment, while urban facilities made up about 25%.
